    Triage Team

    Provides initial assessments & introduces clients to NorthCare and our services. Screeners meet with clients who walk in for the first time or for the first time in a long time to discuss services with them & make sure we are a good fit for them & vice versa. This team also helps clients who present in crisis & scheduling/placing intakes.

    Rapid Response and Outreach

    Assists in providing short-term, intensive services for individuals discharging from inpatient psychiatric treatment with a focus on suicide prevention, reduction in hospital readmission and connection to traditional outpatient services.

    Intensive Transition Team (ITT)

    Assists with engaging those who have recently been hospitalized in outpatient services. This team coordinates discharge through meeting with the client while inpatient, planning for aftercare needs, & linking these clients to outpatient service providers.

  • Psychiatric Evaluations and Medication Services

      NorthCare provides psychiatric evaluations, pharmacological management, and medication training and support for children and adults. Our team consists of board certified psychiatrists and nurses. Individuals seen by the psychiatrist may have medications filled at our pharmacy during their scheduled appointment, if they so choose.  NorthCare is able to assist individuals with applications for various Patient Medication Assistance programs.

      NorthCare also maintains a refill phone line.

      The NorthRock Pharmacy provides integrated, on-site medication and pharmacy services to mental health providers and their consumers to reduce workloads, increase medication compliance, and improve outcomes for those we serve.  Pharmacists work close with the onsite prescribers and medication clinic.  The pharmacy offers mental health and primary care prescriptions.  As part of employee benefits, employees can get their personal prescriptions filled at more competitive prices.

    • Team-Based Care

      As part of our integrated approach to care, NorthCare has a team of qualified staff who work together to ensure our clients receive the services they need and continuity of care across various community resources. We work with community partners and primary care providers to assist our clients in reaching their goal of a healthier lifestyle and overall physical and mental well-being.

    • Criminal Justice Services

      NorthCare has significant experience in providing recovery-oriented services to individuals with substance use or mental illness who are involved in court diversion and re-entry services. NorthCare is an approved specialty court provider and Department of Corrections service provider.

      Drug Court and Mental Health Court

      Works with the jail and court systems to divert non-violent individuals into treatment, help them meet court standards, and set treatment goals for recovery. NorthCare provides treatment and supportive services to both courts in Oklahoma County.

      HERS/Female Diversion

      Provides behavioral health treatment and support to individuals in the female diversion program through the Department of Corrections.

      Veteran’s Diversion Program

      Is designed to address the uniquely complex needs of veterans involved with the criminal justice system in Oklahoma county.

      Re-entry Intensive Care Coordination Teams (RICCT)

      Is an intensive reintegration program designed to facilitate the transition of individuals out of incarceration and into the community.

      Day Reporting

      Provides community monitoring and supportive services to persons incarcerated in the Oklahoma county jail who are released without a cash board and do not pose a serious risk to the community.

      Offender Screenings

      Are provided as a requirement for those applying to a court diversion program (drug court, community sentencing, Female Diversion (HERS), ReMerge, and the Oklahoma County Veteran’s Diversion Program).

    • Program of Assertiveness Treatment (PACT)

      Using a 24 hours a day, seven days a week, team approach, PACT delivers comprehensive community treatment, rehabilitation, and supportive services to clients in their homes, at work, and in community settings. PACT reduces hospitalizations, homelessness, and criminal incarceration.

    • Adult Community Based (ACB)

      ACB provides in home services to adults with high needs who are unable to receive services typically provided in the office.

    • Employment Assistance

      Independent Placement and Support (IPS) is an evidence-based approach to supported employment for people living with behavioral health conditions. IPS supports helps people work at regular jobs of their choosing.

    • Psycho-Social Rehabilitation (PSR)

      PSR is a day-program focused on skill development and social connectedness. The day program provides an environment where every “member” is needed, wanted and welcome, takes control of their recovery, and contributes their unique talents and abilities to the day-to-day functions of the program. This program will also support individuals recently discharged from psychiatric hospitals and individuals who are homeless to have a safe place to go and receive additional supportive and mental health services.

    • Substance Use Services

      Medication Assisted Treatment

      Medication-Assisted Treatment (MAT) is the use of medications, in combination with counseling and behavioral therapies, to provide a “whole-patient” approach to the treatment of substance use disorders. It is also important to address other health conditions during treatment.

      Regional Prevention Coordinators

      Regional Prevention Coordinators (RPC) serve Oklahoma County to reduce the consequences related to substance use & increase the prevention capacity & infrastructure at the community level. These initiatives are currently focused on activating large-scale population-level change to reduce underage drinking, adult binge, and non-medical use of prescription drugs.
