Adult Screening & Intake
The Adult Screening and Intake department serves as the first face of our adult services and strives to match the needs of individuals with internal agency resources and/or community referrals. We foster a welcoming environment as we work with potential consumers to identify their strengths, needs, and preferences. If an individual can be well served by NorthCare, staff will provide timely access mental health and/or co-occurring services. Persons who are interested in receiving services at NorthCare may call the Screening Department or simply walk in our Adult Services location. If an individual is in crisis when they arrive at NorthCare, the Screening and Intake staff will take immediate measures to refer and link the individual with appropriate crisis management resources.

Adult & Senior Home Based Counseling Services
Providing in-home mental health and substance abuse counseling, this program serves adults and seniors, who are at least 21 years old and reside in Oklahoma, Canadian, Cleveland, McClain and Logan counties. These services are provided to individuals unable to leave their homes due to a mental health and/or medical disability, and who are also in imminent risk of psychiatric hospitalization or nursing home placement. Services include assessment, individual and family therapy, crisis intervention, and psychiatric-social rehabilitation. Our staff consists of licensed clinical social workers, rehabilitation specialists and masters-level therapists, who are experienced in treating depression, anxiety, trauma, grief and loss.

Adult Outpatient Counseling
NorthCare’s Adult Outpatient Counseling program serves individuals with emotional, co-occurring or behavioral health conditions through individual, group, family and couple’s counseling. The various therapeutic modalities used include Cognitive-Behavioral, Solution-Focused, Client-Centered and Reality therapies. Our focus is to help consumers achieve a better quality of life, by addressing various issues such as depression, anxiety, grief, loss, sexual or physical abuse, traumatic life events, and thought disorders. Therapy works toward a resolution, recovery of the interpersonal conflict, and improves decision making ability.

Case Management
NorthCare’s Case Management program provides linkage and advocacy services for adults with advanced mental health issues. Service goals of Case Management include: assisting consumers in attaining and maintaining their optimum level of functioning in the community, enabling consumers to adjust to community living by increasing their independence and decreasing psychiatric hospitalization through the provision of support services. Services provided are based on the preferences and needs of each individual.

Community Sentencing
Referrals to the Community Sentencing program are received from Oklahoma County probation officers. Individuals referred to the program are new to the probation system. These individuals receive assessment and treatment for mental health and substance abuse issues. Community Sentencing staff conduct an assessment to identify the individual’s needs before matching them with appropriate treatment services. Services include individual and group counseling for mental health and substance abuse disorders. Program goals are to assist individuals in successfully completing the terms of their probation, and to help them succeed in living independently within the community.

Day Reporting
NorthCare’s Day Reporting program identifies persons incarcerated in the Oklahoma County Detention Center, who are on pre-trial status and eligible for early release with a requirement of services and supervision. A judge from the Oklahoma County District Court must approve the release of all persons to the program. The Day Reporting Center, in conjunction with the Oklahoma County Sheriff’s Department, delivers therapeutic services, while also providing supervision services. By providing treatment plans and services, we attempt to address the mental, physical and emotional needs of the individual so that they may live successfully in the community. A collaborative effort between NorthCare and Oklahoma County Commissioners, the Day Reporting program is designed as an alternative to long term jail incarceration.

Jail Diversion
NorthCare’s Jail Diversion program was created to prevent repeated incarceration of adults exiting the Oklahoma County Detention Center. This program provides discharge planning and reintegration for persons exiting the Detention Center. The program serves adults with serious behavioral health and/or substance abuse disorders. Case Managers connect consumers with integrated and community based behavioral health services to assist them in living independently and successfully within the community.

Program for Assertive Community Treatment (PACT)
PACT provides treatment and case management services for individuals experiencing advanced mental health issues. The program is distinguished from more traditional services by several important features. Instead of a case manager coordinating services for an individual, a multi-disciplinary team provides services tailored to the individual’s specific needs. NorthCare’s PACT team includes members from the fields of psychiatry, nursing psychology, social work, and substance abuse. Also included on the PACT team are Vocational Rehabilitation and Peer Specialists. Based on their various areas of expertise, the team members collaborate to deliver integrated services of the recipient’s choice, monitor progress towards goals, and to adjust services over time to meet the recipient’s changing needs. Services are mobile, delivered by the PACT team to the individual in their own environment, rather than hospital or clinic settings. These services are provided 24 hours-a-day, 7 days-a- week, for as long as they are needed. PACT serves Oklahoma and Payne Counties and is funded in part by the ODMHSAS.

PATH Homeless Program (PATH)
Serving Oklahoma County, PATH identifies and engages chronically homeless people with serious mental illnesses and/or substance abuse disorders. The program works to improve the general health and well-being of individuals by linking resources and services in four categories of need: mental health services, housing, income and basic living needs. PATH seeks to empower individuals by helping them develop the skills, abilities, resources and support necessary to maximize self-sufficiency and independence in the communities where they live. Services are provided within a community setting, including streets, shelters, or any other setting based on the individual’s needs. Individuals may also receive services by visiting NorthCare’s Adult Services location.

Probation Supervision
NorthCare’s Probation Supervision program is designed to assist municipal court systems in supervising misdemeanor non-violent offenders. We are committed to providing courts with options when dealing with offenders. Case managers offer probationed individuals the guidance, services and resources to successfully complete the terms of their probation, while allowing them to remain in the community. Services provided include substance abuse evaluations and drug testing.

Re-entry Intensive Coordination Team (RICCT)
RICCT focuses on the reintegration of incarcerated individuals, who have an advanced mental illness and are detained in an Oklahoma Department of Corrections Secure Facility. Referrals are received when the RICC Team is notified of an individual’s pending release from a correctional institution. The Team makes immediate contact with prison officials to gather pertinent information, and also with the individual to extend the opportunity to voluntarily participate in the program. After an individual is accepted into the program, the RICC Team works to identify services that will assist the individual in making a successful transition into the community. Services may include treatment of co-occurring disorders for persons diagnosed with a mental illness and a substance abuse disorder.

Substance Abuse & Co-occurring Disorders
Our agency provides comprehensive services for adults, who are experiencing substance abuse and co-occurring disorders. These services are based upon abstinence recovery principles. Group and individual counseling are the primary services employed, and family sessions are available upon request. Services are determined by the personal goals of the individual at the onset of treatment. For co-occurring disorders, treatment targets the use of substances in the context of the individual’s mental illness.

Trauma Recovery Groups
NorthCare offers group therapy for women and men, who have experienced trauma. Consumers are provided a step-by-step process and structured interventions to recover from the consequences of trauma. Symptoms of trauma include: avoidance of stimuli associated with the trauma, chronic re-experience of the traumatic event [flashbacks], agitation, and disturbances that cause significant distress or impairment in social, occupational or other areas of functioning. Recovery Groups facilitate the reduction of trauma symptoms by treating the cause in clinician-led and peer supported therapy groups.

Unity House
Unity House is an adult psychiatric rehabilitation program designed to assist individuals in recovery from symptoms of mental illness and addiction through self-directed participation in social, daily living, pre-vocational and educational activities. Over 40 different skills and recovery based classes are offered each week to participants. Services are provided with the belief that individuals with mental health and substance abuse disorders have the right and ability to take charge of their lives and lead valued roles in our society. The program’s goals are to increase each participant’s potential for occupational achievement, goal setting, skills development, and to increase their quality of life. Unity House serves Oklahoma County and is funded by ODMHSAS.
